Please mail or fax the Registration Form (all pages) to: Registration Form: pdf or word Early Registration Fee: $450.00 USD Registration Fee After February 1, 2010: $475.00 USD Onsite Registration Fee: $500.00 USD The conference registration fee pays for the participant only. Includes program materials, conference proceedings publication, evening welcome reception, welcome gift, daily breakfast buffet (for participant only) in hotel restaurant, daily break service, and a mid-week dinner luau buffet. Additional meals may be purchased separately for guests of the participant. Please see registration form for pricing. Cancellation/Refund Policy: After February 1, 2010, absolutely no registration refunds are granted. Requests made in writing and postmarked by February 1, 2010, receive a 75% refund. Telephone cancellations cannot be accepted. ICNS mails refunds one month after the meeting.
